How To Honor Your Pets

1.  You are a client of record with Country Animal Hospital.

2. We need your photos.  If they are on your computer, you can e-mail them directly to our web administrator. If you have hard copies only, you can bring them to our clinic.  If you want to pick the photos up after they have been scanned, please let our receptionist know.

3.  Please be sure to include your pet's name with each photo.

4.  If you wish, include the dates of your pet's birth and/or death.

5.  Please specify if you want both your first and last name included with your pets, your first name only, or your family name.  For example: "Sara" or "Sara Smith" or "The Smith Family".
